In order to make the following training as meaningful as possible, we have worked directly with children during filming. We have received written consent from parents for their children to be involved and for staff working with children in early years settings to watch the videos.

At Speechworks, we take our responsibility to safeguard all children incredibly seriously, as we know you do too.

This form is to be completed by a senior manager or owner of the above setting. Please tick the following agreement and submit. You will then be provided with a unique access code which qualified personnel will be able to use to access the training.

You agree to consent to the following:

* The training videos will only be accessible to staff who have a current enhanced DBS check and will not be visible/accessible to any other members of staff, visitors or parents at any time.

* The Talking Matters training videos will only be accessible to staff working in the above named setting when they are on work the premises in work time. All staff participating in the training will agree to this individually prior to access to the Talking Matters training videos.

* Staff working in the above setting understand their duty to safeguard children. They have agreed not to share the access code or training videos provided with anyone outside of designated colleagues working in the same setting at any time.
